Description

AGHRB award to implement a culturally tailored decentralization programme for snakebite treatment in indigenous communities in the Brazilian Amazonia.

Objectives

Aim 1. Culturally and linguistically translate and adapt our decentralization programme previously developed for the general population. We will engage health care professionals from the indigenous health system, indigenous healers, and indigenous community agents to co-design AJURI-a culturally tailored antivenom decentralization strategy for the indigenous population. Aim 2: Determine the patient-level impact of the implementation of AJURI on time to access SBE antivenom care and functionality. We will conduct a cluster randomized trial in selected CHC facilities with the hypothesis that AJURI lowers time from SBE to antivenom and reduces the envenoming impact on functionality compared to usual care. We will evaluate potential biologic, fidelity, and cultural mediators and moderators. We will also evaluate adoption, implementation, and sustainability outcomes following the Consolidated Framework for Implementation Research Outcomes Addendum. Aim 3: Evaluate the economic and population health impact of the state-level implementation of AJURI. We will use state-level secondary data on SBE time to care and morbidity and mortality to conduct a pre-post AJURI implementation and cost-effectiveness evaluation. We will engage with key personnel at the local, state, and federal level to develop a sustainment plan for AJURI and a path to policy.
